Distinguishing Career Features
The Clinical Case Manager reports to the Manager, LCSW. This position provides direct clinical mental health services to students, including clinical case management. Additionally, as needed the Clinical Case Manager may assist with triage, crisis intervention and assessment, brief individual and group therapy.

Summary

Under the direction of the LCSW, Manager, the Clinical Case Manager will provide clinical case management services, as well as direct clinical social work and mental health services to the college community. The Clinical Case Manager primarily provides direct student mental health services, including case management services, individual and group therapy, clinical triage, crisis intervention, and assessment under supervision. This position will assist LCSW, Manager in developing and maintaining relationships with local community partners and referral sources for students accessing support. Additionally, this position may serve as a resource for mental health prevention and promotion activities including classroom presentations and campus outreach events.

Job Duties

Essential Duties and Responsibilities:
  • Exhibit an equity-minded, trauma-informed focus, responsiveness, and sensitivity to and understanding of the diverse student population of community college students, fostering, and supporting an inclusive approach.
  • Conduct initial clinical case management meetings with students to assess their needs for care, support, and appropriate referrals. Assist with ongoing support to students to achieve collaborative clinical case management goals such as successfully linking to off-campus care, including mental health therapy, psychiatrists, Autism and ADHD evaluations; increasing understanding of health insurance plan coverage and processes for accessing care; enrolling in or re-establishing enrollment in health insurance including Medi-Cal, etc.
  • Provide immediate crisis intervention and continuity of care for students, including assessment of mental status and clinical needs.
  • Develop after-care plans for students with complex mental health needs requiring services after and beyond the counseling and medical care provided by SHS staff.
  • Under the clinical supervision of licensed mental health staff, may provide triage, crisis visit intervention, individual and group therapy to students.
  • Provide timely written documentation in the EMR and assist LCSW, Manager with tracking of clinical case management utilization and outcomes.
  • Participate, under the guidance of LCSW, Manager, in community outreach efforts to educate students and greater college community on mental health and wellness issues, their importance and the availability of resources to provide support.
  • Participate in weekly group and/or individual clinical supervision as needed and within guidelines required for pre-licensed MSW providing clinical social work services.


Minimum Qualifications

Minimum Qualifications for Education and Experience:

  • Graduation from an accredited master's in Social Work degree program.
  • Have registered or will register within 90 days of hire with the California Board of Behavioral Sciences as an Associate Clinical Social Worker (ASW).
  • Experience providing case management, crisis intervention and outreach services.
  • Experience in collaborating with clients from diverse backgrounds and/or those for whom English is not the first language.
  • Understanding of and sensitivity to meeting the needs of the diverse academic, socioeconomic, cultural, disability and ethnic background of the student, community, and employee populations.
  • Work with the core hours of 8:00am to 5:00pm (negotiable), Monday through Friday.

Preferred Qualifications:
  • Experience working in a college mental health services center providing services to diverse students, faculty, and staff.
  • Experience with Point and Click Solutions (PNC), EMR.

Knowledge and Skills
The position requires professional knowledge of:
  • Knowledge of philosophy and guiding principles for effective case management.
  • Knowledge of models, practices, and tools of case management applicable to college students with a wide range of mental health, social services, and other needs.
  • Knowledge of safety screening and planning including assessing risk of suicide, harm to others, child and dependent adult/elder abuse, and interpersonal violence and delivery of crisis intervention/emergency services, and demonstrated crisis intervention skills and techniques.
  • Knowledge of applicable federal, state, and local laws, rules and regulations including FERPA and HIPAA.
  • Knowledge of ethical standards of clinical social work.
  • Knowledge of clinical documentation and recordkeeping rules and guidelines.
The position requires demonstrated skill in:
  • Providing clinical case management services, including assessment, collaborative goal-setting, and referrals for students with a wide range of mental health needs to support their academic success and retention.
  • Providing mental health services to clients from diverse backgrounds and intersectional identities.


Abilities

This position requires the ability to:
  • Conduct clinical case management assessments and other analyses to identify a wide range of care needs for students often with complex mental health needs.
  • Create and follow through on sound continuity of care plans to support student short- and long-term mental health, psychiatric and social service needs.
  • Apply sound crisis management skills when supporting students in high intensity and crisis situations.
  • Work with and assist students navigating through complex systems.
  • Prepare clear, concise, and comprehensive documentation in a timely fashion.
  • Understand and follow applicable legal requirements and ethical standards including seeking consultation as needed.
  • Provide mental health services, including clinical case management, and as needed clinical triage, brief individual and group therapy, crisis visit interventions and mental health assessment and referral services to students with a wide range of mental health needs to support their academic success and retention.
  • Analyze situations accurately and adopting an effective course of action.
  • Continuously engage in learning and professional development.
  • Work cooperatively and productively with internal and external constituencies.


Licenses and Certificates
  • Master's in social work from an accredited master's degree program.
  • Registered, or application in process, as an Associate Clinical Social Worker (ASW) with the California Board of Behavioral Sciences within 90 days of hire. *Supervision of clinical hours toward licensure available.

Salary:
$30.00//hr.
